Eastern Iowa Health Center (EIHC) has an exciting opportunity for a Registered Nurse (RN) to join our team of healthcare professionals at our family medicine/pediatric location. The ideal candidate must have a commitment to providing high-quality primary care for our complex patients. Under the general supervision of the Operations Manager and Clinic Administrator, the RN shall evaluate and coordinate comprehensive care for patients of all genders, cultures, and backgrounds.

Responsibilities

  • Oversee clinical care of other personnel, including the Licensed Practical Nurse and Medical Assistant.
  • Provide individualized education to patients and families appropriate for age, diagnosis, and level of understanding.
  • Provide general nursing care and leadership, working collaboratively with physicians and multidisciplinary team members.
  • Assist physicians with patient office visits, including rooming and interviewing patients, recording vital signs, and entering information in patient’s electronic medical record.
  • Perform select clinical tests including, but not limited to: blood glucose, rapid HIV testing, influenza, and strep cultures. Manage and operate equipment safely and correctly; correctly labels all specimens.
  • Observe, assesses, records, and reports changes in patients’ condition.
  • Administer medications according to orders from a Licensed Independent Practitioner (LIP), following the six rights of medication administration.
  • Assist providers with procedures by providing pre- and post-procedural care, participating in the Time Out, and intra-procedural support.
  • Screen patient calls and make follow-up calls as requested by the provider.

Required Qualifications

  • Current RN license recognized by the State of Iowa and in good standing
  • Current Basic Life Support for Health Care Providers Certification (BLS)
  • Electronic health record experience, Athena preferred

Preferred Qualifications

  • Previous experience in a medical clinic
  • Advanced working knowledge of outpatient care, triage, and clinic flow
  • Demonstrated ability and sensitivity to persons who are disabled, homeless, substance users, HIV (AIDS) infected, and/or psychologically impaired

Eastern Iowa Health Center (EIHC) has an exciting opportunity for a Certified Medical Assistant (CMA)/Licensed Practical Nurse (LPN) to join our team of healthcare professionals at our family medicine/pediatric location. The ideal candidate must have a commitment to providing high-quality primary care for our complex patients. Under the general supervision of the Operations Manager and Clinic Administrator, the CMA/LPN shall be knowledgeable of the standard practices, rules, and regulations related to respective licensure.

Responsibilities

  • Provide general medical care and select patient care tasks while working collaboratively with physicians, nurses, and other multidisciplinary team members.
  • Provide individualized education to patients and families appropriate for age, diagnosis, and level of understanding with the direction of RN or Licensed Independent Practitioner.
  • Assist providers with patient office visits, including rooming and interviewing patients, recording vital signs and entering information in patient’s electronic medical record.
  • Responsible for maintaining organized and clean exam rooms and stocking with required supplies and equipment for each visit.
  • Perform select clinical tests including, but not limited to; blood glucose, rapid HIV testing, influenza, and strep cultures.
  • Manage and operates equipment safely and correctly. Observes, records, and reports changes in patient’s condition.
  • Administer medications according to orders from a Licensed Independent Practitioner (LIP), following the six rights of medication administration; including maintenance of intravenous lines.
  • Assist providers with procedures by providing pre-procedural and post-procedural care, participating in the Time Out, and intra-procedural support.
  • Screen patient calls and make follow-up calls as requested by the provider. Direct calls requiring assessment to an RN or LIP.

Required Qualifications

  • Completion of Medical Assistant program at an accredited institution  
  • Current LPN license in the State of Iowa & in good standing
  • Current Basic Life Support for Health Care Providers Certification (BLS)
  • Electronic health record experience, Athena preferred

Homeless Outreach Social Worker

Homeless Outreach/Medical Social Worker (Full-Time)

Eastern Iowa Health Center (EIHC) has an exciting opportunity for a full-time Homeless Outreach Social Worker to join our team of healthcare professionals at our family medicine/pediatric location.

​The Homeless Outreach Social Worker is responsible for strengthening and supporting civic and community organizations working for the betterment of the homeless community, as served by EIHC. Additionally, you will be responsible for providing patient psychosocial assessments to determine treatment plans.

This position will also help connect patients and families to necessary resources and supports in the community and make appropriate referrals as needed. This position will build and maintain partnerships with local organizations that expand the reach of EIHC’s mission. This position implements EIHC activities and outreach focused on the homeless population, both internally and externally. In addition, this position is responsible for other outreach duties, including but not limited to: representing EIHC at health fairs, roundtables and National Health Center Week activities.

Responsibilities​

  • Initiate contact and manage relationships with service and community organizations, businesses, healthcare organizations and providers, academic institutions, as well as local and regional governmental agencies in order to further the services to the homeless population
  • Identify ways to engage the community by establishing relationships with formal and informal leadership and neighborhood organizations to facilitate awareness and utilization of EIHC and its services, specifically for the area homeless population
  • Practice Trauma-Informed Care
  • Provide emotional support and brief interventions, based on individual client needs
  • Provide direct service to clients and their relatives through the evaluation of social and psychological data and select appropriate social work methods for treatment
  • Consult with clinical staff to assess the medical, psychological and social condition and needs of client, as necessary
  • Assess high-risk situations/symptoms regarding suicide potential, domestic violence and child/dependent abuse

Required Qualifications

  • Bachelor’s degree in Social Work (BSW) or relevant field of study
  • Licensed in Iowa as LBSW
  • Valid Iowa driver’s license, proof of insurance and own transportation
  • Demonstrated abilities to communicate with a variety of audiences, along with multi-tasking and time management skills
  • Basic computer skills required.
  • Current Basic Life Support for Health Care Providers Certification (BLS)
