If you qualify, the amount you pay for eligible services is reduced based on your income and household size.
A Patient Representative can help you understand the program, gather the information you need and complete the enrollment process.
Cost should not prevent you from seeking healthcare when you need it. Eastern Iowa Health Center offers a Sliding Fee Discount Program to help make healthcare more affordable for eligible patients.
Eastern Iowa Health Center is not a free clinic. However, qualifying patients may receive discounted services based on household size and gross household income.
You may apply even if you have health insurance. Our team can help you understand whether you may qualify and what the discount means for your care.
Eligibility for the Sliding Fee Discount Program is based on:
Your household size
Your gross household income
If your income or household size changes during the year, you may ask to have your eligibility reviewed again.
Sliding fee discounts are generally effective for one year. You will need to reapply when your eligibility period expires.
Once you become an Eastern Iowa Health Center patient, a Patient Representative can meet with you and help you apply for the Sliding Fee Discount Program.
You will be asked to provide information about your household and income so we can determine whether you qualify and the amount of your discount.
You may be asked to provide documentation showing your household income. Depending on your situation, acceptable documentation may include:
Recent paycheck stubs
W-2 or other wage documentation
Most recent federal income tax return
Social Security or disability benefit statements
Retirement or pension benefit statements
Unemployment benefit statements
Workers’ compensation benefits
Veterans benefits
Child support or alimony documentation
Military family allotments
Other documentation showing household income

The amount you pay depends on your eligibility for the Sliding Fee Discount Program and the services you receive.
For patients receiving a sliding fee discount, the minimum charge is currently $25 for Medical, Dental, Behavioral Health, Pediatrics and OB/GYN services.
If you are unable to pay your bill or have questions about what you owe, please contact our Billing Department. We want to help you understand your options.
Eastern Iowa Health Center accepts Medicaid, Medicare, Iowa Health and Wellness and many private insurance plans.
Having insurance does not necessarily mean you should skip applying for the Sliding Fee Discount Program. If you are concerned about the cost of your care, talk with our team about whether you may qualify.
