The sliding fee discount program allows us to reduce or “slide” the fees for you or your family’s care at EIHC. You may apply for the program to see if you qualify for a discount.
Eligibility is based on gross income and family size. If the income or family size changes, you may re-verify at any time throughout the year.
HOW TO APPLY: Once you become a patient at EIHC, a Patient Representative will be available to meet with you and help you enroll in the sliding fee program. Sliding fee discounts are generally effective for one year. Once the discount expires, you will have to re-apply.
In order for an application to be processed and approved, persons applying must provide an acceptable from documentation:
How much do I owe? Your cost will always be at least $25 for Medical, Dental, Behavioral Health, Pediatrics, and OB/GYN services. If you are unable to pay your bill, please contact the Billing Department at 319.730.7331.

Payment is expected at the time of your visit. The cost includes in-house lab fees.
Yes. The discount will be applied after any insurance payments. This includes private Insurance, Medicare, and Medicaid.
Grants and donations are received to help offset costs.
You can still apply for discount. A Patient Representative will be able to assist you. You will still be charged the nominal fee for your visit. You can make any amount of payment to keep you account current.
Yes. All information collected from you is confidential.
Yes. EIHC has dedicated staff to help enroll patients into Medicaid and Marketplace.
You can contact the billing Department at 319.730.7331. They are able to help you pay your bill, create a payment plan, or available for any concerns or questions you may have.
