WHEN: September 6, 2023    4 PM – 5:30 PM
WHERE: Mercy Hospital 701 10th St SE Cedar Rapids, IA (2nd Floor)
HOW TO SCHEDULE: Fill out the form and we will call you to schedule you appointment. 
WHEN: October 12th, 2023    4 PM – 5:30 PM
WHERE: PCI Medical Pavilion- 202 10th St SE Cedar Rapids, IA 52403 (Enter through 3rd St.)
 
HOW TO SCHEDULE: Fill out the form and we will call you to schedule you appointment. 
The Care for Yourself program provides free breast and cervical cancer screenings and exams to those that qualify are uninsured or underinsured. To qualify, you must be between 21-64 years of age and have a monthly income of $3,038 or less (add $1,070 per family member).
If you are unable to make this event, please reach out to us to enroll in the Care for Yourself program for a mammogram that better fits your schedule.
