Making the Appointment

Call the main number 319.730.7300 to schedule an appointment.When you call the number you will be given options to choose from. Please choose option #1; this will direct your call to our schedulers.
If the schedulers are busy, a message or music will be heard until they can take your call.

Message Nurse

LCC provides a telephone triage nurse to help answer your questions, get refills for prescriptions, and get messages to your assigned Clinician. The message nurse in our office is available to you Monday  through Friday between the hours of 8 AM to 5 PM by choosing  option #2.
LCCAn answering service will take calls during off hours.

  
Computers
LCC Clinicians and staff use the electronic medical record  as your chart. The electronic medical record is a useful and important tool in the medical office setting. This resource gives us the ability to give you the most up-to-date quality health care. One very big advantage of this tool is that any time you call our office, the nurse or clinician can look at your chart immediately.
LCC
Laboratory Services

LCC has full-time Lab Assistants.We have a CLIA certified laboratory. We do simple lab tests here and can also draw your blood for tests and send them to Quest Laboratories for interpretation.You may also use St. Luke’s or Mercy Hospital labs, as well as any of the other lab facilitiesin Cedar Rapids.

Prenatal Care Program
LCC’s goal is to provide prenatal care to women in their first trimester. Women who seek early entry to prenatal care reduce the rate of infant mortality. Having healthier moms creates healthier babies.

Mental Health
At this time limited mental health services are provided in-house. Most cases are referred outside to collaborating medical and community agencies.We will work closely with those outside providers to ensure your care is coordinated appropriately.


Oral Health

LCC is committed to making sure you are healthy in every way possible.We currently provide limited adult dental care through community dentists for those patients who qualify. If you are in need of dental care, but finances have been a barrier, please talk to our Patient Advocate to see if you qualify for our adult dental program.

LCCLCCMedication Assistance Program
LCC offers several prescription assistance options to its uninsured and underinsured patients, through various pharmaceutical companies. LCC also offers low-cost prescriptions to its patients through a local pharmacy, 5th Avenue Pharmacy at 411 10th Street Southeast.

Medical Records
Our computerized record is your “chart” at LCC.We take seriously the need to keep your medical information confidential. Any request for medical information or transfer of medical records must have a Release of Information form signed by you the patient, or your legal representative.

LCCFinancial Information
LCCLCC accepts all insurances and also has a Sliding Fee Scale available to our patients. Please feel free to speak with our Patient Advocate to help you determine what assistance you may qualify for. Our billing staff will also be happy to answer questions about your bill at any time. Please choose option
#3 on the phone line.

Translation Services
LCC provides translation services to its non- English speaking patients.

Transportation

If you have difficulty finding transportation to and from your appointments at Linn Community Care, please let us know. We will assist you.
